What Is Rotablation Angioplasty?
Rotablation is a special angioplasty for very hard, calcified blockages. A tiny diamond-coated burr spins at high speed to drill and sand away the hard calcium, so that a balloon and stent can then open the artery.
Why Is Rotablation Angioplasty Done? — Indications
It is done when calcium has made an artery too hard or stiff for a balloon or stent to pass through or expand properly.
Who May Need Rotablation Angioplasty?
It is for patients with heavily calcified coronary arteries that are not suitable for routine angioplasty.
Types / Variations of Rotablation Angioplasty
Rotational atherectomy (Rotablation)
A diamond-tipped burr spinning at very high speed sands away hard calcium.
Cutting / scoring balloons
Balloons with small blades or wires that crack calcium in less severe cases.
Intravascular lithotripsy
Sound-wave pulses that crack calcium inside the artery wall — another option for calcified blockages.
Your cardiologist will tell you which type or technique is right for you after reviewing your reports.
Benefits of Rotablation Angioplasty
It makes stenting possible in hard, calcified arteries that could not otherwise be treated without surgery.
How Rotablation Angioplasty Is Performed
It is done through the wrist or groin in the cath lab. The rotablator burr is advanced over a wire to grind away the calcium, followed by a balloon and stent.
Preparation Before Rotablation Angioplasty
Preparation is the same as for angioplasty, with detailed imaging to assess how much calcium is present.
Tests Required Before Rotablation Angioplasty
- Coronary angiography — the images used to plan the procedure
- Blood tests — haemoglobin, kidney function (creatinine), blood sugar and clotting profile
- ECG — to record the heart's rhythm on the day
- 2D Echo — to check pumping (EF) and the valves
- Infection screening (HIV, hepatitis B and C) as per hospital protocol
- IVUS during the procedure, in many cases — to see how much calcium is present
Your doctor will tell you exactly which tests you need — bring all previous reports, CDs and your medicine list.
Anaesthesia / Sedation Used in Rotablation Angioplasty
Local anaesthesia, usually with mild sedation for comfort. You remain breathing on your own.
How Long Does Rotablation Angioplasty Take?
Usually about 1–2 hours, including stenting.
Hospital Stay / Admission for Rotablation Angioplasty
Usually 1–3 days.
Risks & Possible Complications of Rotablation Angioplasty
Risks are slightly higher because of the complexity, so it is done by experienced operators to keep risk to a minimum.
Recovery After Rotablation Angioplasty
Recovery is similar to standard angioplasty, with a short hospital stay.
Aftercare & Precautions After Rotablation Angioplasty
- Take every medicine exactly as prescribed — never stop blood thinners on your own.
- Keep the puncture site clean and dry for 2–3 days; a little bruising is normal.
- Avoid heavy lifting and strenuous work for the first few days, as advised.
- Drink plenty of water for 24 hours to flush out the dye (unless told to limit fluids).
- Control BP, sugar and cholesterol, stop tobacco completely and eat home-cooked, low-oil food.
- Keep every follow-up visit and bring your discharge summary and reports.
Success / Expected Outcomes of Rotablation Angioplasty
Rotablation makes it possible to place and fully expand a stent in arteries that are too hard for a balloon alone, giving lasting relief of symptoms in most suitable patients.
When to Seek Medical Help After Rotablation Angioplasty
Contact us immediately — or go to the nearest emergency — if you notice any of these after going home:
- Chest pain, pressure or tightness — especially at rest
- Bleeding, swelling or increasing pain at the puncture site — press firmly on it and call us
- A cold, pale, numb or painful hand or leg on the puncture side
- Breathlessness, or swelling of the feet that is getting worse
- Passing much less urine than usual
Rotablation Angioplasty — Questions Patients Ask
It is done under local anaesthesia like other angioplasty; you stay awake and comfortable.
Hard calcium stops balloons and stents from expanding. Rotablation removes it first.
Only in selected heavily calcified cases, as decided by the cardiologist.
