What Is CRT-P / CRT-D?
CRT (Cardiac Resynchronisation Therapy) is a special pacemaker that helps the heart’s chambers pump together in patients with heart failure. CRT-P is the pacemaker type; CRT-D also has a defibrillator (shock) function.
Why Is CRT-P / CRT-D Implantation Done? — Indications
It is done in heart failure where the heart’s chambers beat out of sync; CRT helps them pump together and improves symptoms.
Who May Need CRT-P / CRT-D Implantation?
It is for heart-failure patients with a weak heart and an electrical delay (seen as a wide QRS or LBBB on the ECG).
Types / Variations of CRT-P / CRT-D Implantation
CRT-P (pacemaker)
Resynchronises the heart's pumping — for patients who do not need shock protection.
CRT-D (defibrillator)
Resynchronises pumping and also gives a life-saving shock for dangerous fast rhythms.
Conduction-system pacing
In some patients, pacing the heart's natural wiring is used as an alternative way to resynchronise.
Your cardiologist will tell you which type or technique is right for you after reviewing your reports.
Benefits of CRT-P / CRT-D Implantation
It reduces breathlessness and tiredness, improves the heart’s pumping and quality of life; CRT-D also protects against sudden death.
How CRT-P / CRT-D Implantation Is Performed
It is similar to a pacemaker but with an extra lead to the left side of the heart, placed under local anaesthesia near the collarbone.
Preparation Before CRT-P / CRT-D Implantation
Preparation includes echo, ECG and heart-function tests, along with the usual device preparation.
Tests Required Before CRT-P / CRT-D Implantation
- 2D Echo — to check pumping (EF) and the valves
- ECG — to measure the QRS width / LBBB
- Holter monitoring, if advised
- Coronary angiography — the images used to plan the procedure
- Blood tests — haemoglobin, kidney function (creatinine), blood sugar and clotting profile
Your doctor will tell you exactly which tests you need — bring all previous reports, CDs and your medicine list.
Anaesthesia / Sedation Used in CRT-P / CRT-D Implantation
Local anaesthesia with sedation. A short deeper sedation is given if the defibrillator is tested.
How Long Does CRT-P / CRT-D Implantation Take?
Usually about 2–3 hours, because of the extra lead to the left side of the heart.
Hospital Stay / Admission for CRT-P / CRT-D Implantation
Usually 1–3 days.
Risks & Possible Complications of CRT-P / CRT-D Implantation
Risks are similar to a pacemaker or ICD, with slightly more complex lead placement.
Recovery After CRT-P / CRT-D Implantation
The hospital stay is short; take arm-movement care for a few weeks and continue regular device and heart-failure follow-up.
Aftercare & Precautions After CRT-P / CRT-D Implantation
- Do not lift the arm on the device side above shoulder level for about 4–6 weeks, and keep the wound dry until it is checked.
- Keep mobile phones and magnets about 15 cm away from the device, and carry your device card (show it at airport security).
- Attend regular device checks — the battery and leads are tested at every visit.
- Take every medicine exactly as prescribed — never stop blood thinners on your own.
- Weigh yourself daily and follow your fluid and salt advice for heart failure.
- Keep every follow-up visit and bring your discharge summary and reports.
Success / Expected Outcomes of CRT-P / CRT-D Implantation
In well-selected patients, CRT reduces breathlessness and tiredness, improves the heart's pumping over months, and lowers hospital admissions for heart failure. Most benefit is seen within 3–6 months.
When to Seek Medical Help After CRT-P / CRT-D Implantation
Contact us immediately — or go to the nearest emergency — if you notice any of these after going home:
- Breathlessness, or swelling of the feet that is getting worse
- Any shock from the ICD — call us; more than one shock, or a shock with feeling unwell, is an emergency
- Continuous hiccups or twitching of the chest or tummy muscles
- Fainting, dizziness, or a very fast or very slow heartbeat
- Fever, or redness, warmth or discharge at the wound
